Please have the completed forms and any medications or equipment brought to the school your child will be attending in the fall as soon as possible. 

Please bring them during our Mann Colt Corral on August 29th, 2019 from 12p - 2p. 

This is to ensure the complete nursing care plan and train/distribute to staff members who must be aware of your child’s health plan.

We want our school to be a safe and healthy place for your child to attend. State Law mandates that we must be prepared to handle an emergency that could arise on the first day of school.
